J&J Medical Supply, LLC (“J&J Medical Supply,” “we,” “us,” or “our”) provides durable medical equipment and related supplies to patients throughout Los Angeles County and Orange County. In fulfilling medical orders, we create and receive records containing Protected Health Information (PHI)—including physician prescriptions, diagnosis codes, face sheets, and insurance identifiers needed to deliver and bill for your equipment. We are required by federal and California law to protect the privacy of this information and to follow the terms of this notice.

Your Health Information Rights

Under federal HIPAA regulations and California medical privacy statutes, you have the right to:

• Inspect and Copy Records: Request an electronic or physical copy of your medical billing and equipment records. We provide this within 30 days in accordance with statutory guidelines.
• Request Corrections: Request amendments to health or billing information you believe is inaccurate. All requests will be evaluated and responded to in writing.
• Confidential Communications: Direct us to contact you at a specific phone number, address, or through a designated caregiver.
• Restrictions on Disclosure: Request limits on how we share your information with family or care coordinators.
• Accounting of Disclosures: Request an accounting of disclosures of your health records made over the past six years (outside of routine treatment, payment, and healthcare operations).
• Paper Copy: Receive a physical copy of this notice at any time upon request, even if received electronically.
• Breach Notification: Be notified promptly in the event of an unauthorized acquisition, access, or breach of unencrypted health information.

Permitted Uses and Disclosures

We utilize your Protected Health Information strictly for clinical, administrative, and claims fulfillment purposes:

• Treatment & Equipment Delivery: We share order specifications with our certified technicians, your prescribing physician, and home health nurses to coordinate equipment setup, sizing, and safety instruction.
• Payment & Insurance Adjudication: We submit physician orders, Certificates of Medical Necessity (CMN), and diagnosis codes to Medicare Part B, California Medi-Cal, or commercial payers to verify eligibility and process payment.
• Healthcare Operations: We review service records to audit inventory, evaluate technician performance, and meet accreditation safety standards.
• Legal & Regulatory Compliance: We disclose documentation when mandated by CMS audit contractors, California Department of Public Health inspections, workers' compensation protocols, or valid judicial subpoenas.
